Recording Forms
The Infertility Assistant is the only infertility resource specifically designed with tables, for side by side comparisons of doctor and clinic details, and ample space for tracking up to six ART procedures of eight weeks duration. There are also forms to keep track of medical history, tests and procedures, medications, comparisons for treatment options, finances, appointments, contact information, and resources.

Other Design Features
The Infertility Assistant is spiral bound and is designed to lay flat or fold over on any page. Tabs help you to navigate to the form or chapter you need quickly. A folder pocket inside the back cover provides a place to hold prescriptions, physician notes, test results, and other important information. Space is provided at the end of each chapter and recording form for jotting notes or questions and keeping track of information you would like to highlight. Flowcharts of general infertility testing and treatments are provided for a simple overview.
